The New York State Amateur Hockey Association, in order to be in compliance with the Affiliate Agreement with USA Hockey, has instituted a policy to screen coaches, on-ice officials and volunteers who have contact with players, whether girls or boys, under the age of 18.  This includes players and coaches who are over 18 playing or coaching on teams containing under 18 year old players, i.e. 19 & Under Women’s teams.

• ALL SCREENING FORMS MUST BE TURNED IN TO SUE PALMER BY OCTOBER 25th EITHER BY MAIL, FAX OR EMAIL. FAILURE TO DO SO CAN SUSPEND YOU AS A COACH/MANAGER.
• Please make sure your name is on the certificate or if emailed to me the name and certificate #.
• All coaches and managers need to comply with this new procedure.
• Any coaching staff must comply : "Coaching Staff" consists of those adults who routinely come into direct contact with a "youth" players.  Therefore, any parent who fits this description must be screened.  Coaches/Managers are to gather these screening certificates and send in also.
• Minors under 18 who assist on teams are not to be screened.
• An adult not directly and routinely interact with the players do not require screening.  Examples,:  score keepers, schedulers, carpooling.

COST:  $13.00 and is good for 3 years.

Please keep a copy of the Certificate # - this Certificate # will be asked for every 3 years so we can re-certify you as coaches/mangers.

Additional fees can be applicable if the Screening Company finds something against you.  You will be notified by the Screening Company that your charge card is being charged.

Any coaches who are affiliated with High School teams can give this Certificate # to them.  You do not need to re-do the screening process.  The Certificate # carries on with you and is good for 3 years.
